Story

FHC's Value Statement
The values of Flemingdon Health Centre are consistent with the Pan American Health Organization (PAHO) / World Health Organization’s (WHO) Declaration of Montevideo. Representatives from all WHO member countries met in Montevideo, South America and reaffirmed the importance of equity and the right of every human being to enjoy the highest attainable standard of health without distinction of race, religion, political belief, economic or social condition. The Declaration came into effect September 2005.

Mission

Focusing on the factors that affect health, Flemingdon Health Centre works with our clients and the diverse community to meet their primary health care needs. Committed to health promotion and illness prevention, we enable social action initiatives and community development. Through our affiliation with the academic community, we participate in teaching and other academic activities.

Impressum

The South Asian Diabetes Prevention Program (SADPP) is an innovative project developed by Flemingdon Health Centre (FHC) in collaboration with Social Services Network and is funded by the Ministry of Health and Long-Term Care through the Ontario Diabetes Strategy Toronto Central Local Health Integration Network (TC-LHIN).

The literature on diabetes in the South Asian population clearly identifies the high-risk status of this group, which is primarily attributable to genetics, lifestyle risk factors, and the stress related to migration.

South Asians are 4-6 times more likely to develop diabetes than the general population: incidence rate of diabetes is 14% among South Asians versus 5.2% in the general population. In addition, South Asian communities often experience challenges accessing healthcare services due to language and cultural barriers.

Thus, the overall goal of the South Asian Diabetes Prevention Program is to enhance equitable access to diabetes prevention and management services as well as provide culturally-relevant and language-specific educational resources for South Asian communities within the North East region of Toronto.
