Story

We are a non-profit, community-governed organization that provides primary health care, health promotion, and community development services using interprofessional teams of health and social service providers. Our dedicated teams include physicians, nurse practitioners, nurses, social workers, dietitians, health promoters, early childhood educators, counsellors and outreach workers.

We are governed by community members and work hand in hand with the people we serve to improve the health and wellbeing of populations who’ve traditionally faced barriers accessing health services.

Mission

We support people and communities to enjoy the best possible health and well-being.

We do this by providing primary health care and social services, and promoting access to the social conditions that influence health, such as housing, food security, employment and civic engagement.

We remove barriers to accessing services for people who are vulnerable because of their age, income, abilities, sexual orientation or gender identity, and language or culture.
