Care Resources is a Program of All-inclusive Care for the Elderly (PACE®). Care Resources was developed as a health care program for older adults needing long-term care and wishing to age in place. As we age, medical problems become more complex and last longer. Care Resources is responsible for all primary and specialty care for those who are enrolled.
PACE is different from any long-term care option. We partner with you to stay independent and in good health, as you age. We have health care professionals who watch for changes in your condition, provide treatment, and encourage you to stay independent. When you enroll in the program, you will receive services unique to your own needs. The PACE model is an all-inclusive health care program that promotes better care and better health.
PACE Eligibility:
- 55 years old or older
- Resident of Kent County or the following zip codes in
Ottawa County: 49418, 49428, 49435, 49534, 49544
- Able to live safely in the community when you join PACE
- Meet the State of Michigan nursing facility level of care*
