MCHC is a nonprofit primary care facility offering high-quality, comprehensive primary and preventive healthcare that is both convenient and affordable. Several options are available for payment of healthcare services and our staff are ready to assist you in choosing what best suits your needs. From full coverage through a commercial insurance program to financial assistance for those who qualify, there’s an option to fit everyone’s budget.
The Health Center accepts most insurance plans including Medicaid, Medicare, and
commercial HMO’s as well as offering a discount sliding-fee scale for those who are
uninsured. A discount on services for those who qualify is based on the patient’s
income and family size.

Primary Care/Family Medicine
MCH’s core service, primary healthcare, is available to all regardless of age, economic status, national origin, disability, ethnicity, creed or sexual orientation. Comprehensive Primary Care is provided by Board Certified Family Practice Physicians, Pediatricians, Nurse Practitioners and Certified Physician Assistants.
Adolescent Preventive Services... 935-5207
MCHC’s Adolescent Preventive Services (APS) program is a preventive health initiative that focuses on youth in Manchester’s public school system. A Licensed Clinical Social Worker provides at-risk students from the middle and high-schools, preventive health education, case management services and on-going mentoring.
Breast & Cervical Cancer Screening... 935-5243
This program serves women with limited income and poor or no health insurance. Screenings are performed annually for those who qualify.
Your primary care provider can refer you to this program as appropriate.
Counseling
Comprehensive assessments, referrals, case management, crisis intervention and counseling services are provided on-site, by licensed practitioners. Talk to your primary care provider if you are interested in counseling services.

Diabetes Education... 935-5243
A Certified Diabetic Educator (CDE) provides diabetic education on an individual basis. MCHC offers glucometer education, diabetic assessments and the newest updated diabetic education. If you are diabetic and would like to learn more, please notify your provider or call MCHC’s CDE.
Diabetic Eye Screening
A licensed Optometrist offers Dilated Retinal Exams for MCHC diabetic patients during regularly scheduled Diabetic Eye Clinics. Your primary care provider will refer you to the Diabetic Eye Clinic as appropriate.

The Diabetic Support Group is open to all patients with diabetes who want to learn, share and grow. Contact MCHC’s Certified Diabetic Educator to learn more.
Lab Services
Elliot Hospital provides lab services on-site at MCHC. Your primary care provider
will refer you to any lab services that are appropriate to your care.
Language Interpretation
MCHC provides interpretation services through onsite interpreters as well as contracting with local community interpreters certified in medical interpretation. More than 65 different languages are spoken at MCHC.
Medicaid Enrollment... 935-5230
A Medicaid Enrollment Coordinator offers onsite assistance with enrollment into NH Medicaid programs for pregnant women and the State’s Healthy Kids programs for children.

MCHC provides comprehensive prescription assistance to eligible patients through its Discount Pharmaceutical Program. The Health Center ensures high-quality prescription medications at a substantially reduced cost.
Nutrition... 935-5246
MCHC has an extensive Medical Nutrition Therapy program which encompasses diabetes care, obesity prevention, prenatal nutrition, as well as counseling for high cholesterol and hypertensive patients.
Pediatric Primary Care
Preventive and acute care is available for children and adolescents (from birth to 18 years) by a
board-certified Pediatrician as well as Family Practice Physicians.
Podiatry
Specialty Podiatry services are available to MCHC patients with a referral from their primary care provider.
Prenatal Care... 935-5234
Prenatal care at MCHC is provided through a team approach consisting of family practice physicians and a Certified Nurse Midwife as well as a dedicated perinatal nurse, a nutritionist and counselors. If you are pregnant, or think you might be pregnant, contact MCHC’s Perinatal Nurse for assistance.
