Products • Long-Term Skilled Nursing Care • NHCA centers have also specialized in delivering quality long-term care to residents. The team develops a comprehensive care plan which includes all medical, nursing, rehabilitation, nutritional, psychosocial and therapeutic needs. Each resident is also evaluated upon admission by our interdisciplinary team, based on need for rehabilitative services in physical, occupational and/or speech therapy. Thereafter, a team of therapists carry out restorative or maintenance therapies in conjunction with the resident’s plan of care. Our goal is to assist each resident with the means to maintain his/her optimum level of functioning. • Passport Rehabilitation • “Clinical Excellence” The Passport Rehabilitation program is a natural progression of our services, helping our short-term rehabilitation patients return to the best quality of life possible, as quickly as possible. The program optimizes recovery through individualized occupational, physical and speech language therapy plans with goals tailored to their specific needs and preferences. We employ trained specialists equipped with a diverse set of skills who are ready to field the most unique challenges in your rehabilitation process. With the Passport Rehabilitation program, our ultimate goal is to restore you to health. We understand that every person under our care has distinct requirements for therapy. Your individual rehabilitation is tailored to a series of realistic, short-term achievements based on getting you home. We encourage you to meet with our interdisciplinary team of specialists to discuss your goals and expectations prior to your stay. • Alzheimer/Dementia Special Care • This program is designed to meet the special needs of Alzheimer’s/Dementia residents in a safe, caring and secure environment. Our goal is to help residents with Alzheimer’s or related dementias maintain their current level of functioning. • Ventilator/Pulmonary Unit • - Four levels of pulmonary care Ventilator Program Tracheostomy Program Short-term pulmonary Rehab Long-term pulmonary Rehab - Self-contained, new state of the art unit Piped in oxygen/suction LTV 1200 ventilators (high performance/compact) - Promoting maximal functional/psychological independence Priority that all patients are out of bed as tolerated for dining, showering and activities - Respiratory Therapy 24/7 3 respiratory therapists per shift Active ventilator weaning protocol Tracheostomy decannulation protocol - Medical Director is Primary Care Physician - Pulmonology rounds, 3x week for ventilator patients, on-site consults for pulm rehab programs - On-site specialty physician consults - Interdisciplinary team approach Physical Therapy Occupational Therapy Speech Therapy, including vital stim program Psychology, evaluations on all new admissions Social Work, support groups for patients/ families Dietary, small meal size and frequency for patients with poor endurance Nursing, Certified Rehab Nurses Recreational Therapists Pulmonary Physician Assistant - Working in collaboration with North Shore Long Island Jewish Heart Smart Program
