About We are a team of healthcare professionals, working collaboratively with family physicians to improve primary care for our patients. Story The Sherwood Park PCN was established in 2007 to help physicians take better care of patients. We are a joint venture between family physicians in Sherwood Park and Alberta Health services. Over 70 family physicians are members of the PCN in over 20 clinics We offer programs that assist family physicians in helping patients with: • Chronic Disease Management • Mental Health We also offer a variety of free classes open to the public that cover nutrition, mental health and exercise.
