Story We have been providing services since 1969. Originally, known as Three Rivers Neighborhood Health Services and founded by the Allen County Medical Society as a voluntary health service to provide well child services to low-income children of Allen County. Over the years we have grown substantially and now offer full primary medical, dental, and optometry care. We have two offices: 1717 S. Calhoun Street & 3350 E. Paulding Road.
